چکیده:
سابقه و هدف: در دهه های اخیر در پی درمان آنتی بیوتیکی، سودوموناس آئروجینوزا به عنوان یکی از مهمترین پاتوژنهای بیمارستانی شناسایی شد. به دلیل اهمیت کلینیکی سودوموناس آئروجینوزا روشهای متنوعی برای شناسایی آن به وجود آمده است. هدف از این مطالعه شناسایی سویه های سودوموناس آئروجینوزا جدا شده از عفونتهای زخم و سوختگی بر اساس PCR ژن های اختصاصی لیپوپروتئینهای غشای خارجی (oprI و (oprLو اگزوتوکسین A (toxA) و تعیین فراوانی ژنهای ویرولانس آنزیمهای نورآمینیداز (nanI) و اگزوآنزیم S (exoS) در این سویه ها است.
مواد و روشها: این مطالعه با طراحی توصیفی مقطعی انجام شد. 150 نمونه سودوموناس آئروجینوزا از عفونتهای زخم و سوختگی از بیمارستانهای امام خمینی، مطهری و توحید در تهران جمع آوری شدند. DNA این سویه ها با روش فنل- کلروفرم استخراج و PCR ژنهای oprI، oprL،toxA ، exoS و nanI با استفاده از پرایمرهای اختصاصی انجام شد. یافته ها: همه 150 سویه سودوموناس آئروجینوزا مورد مطالعه دارای ژنهای oprI و oprL بودند. 142(7/94%) سویه دارای toxA، 98 سویه (33/65%) دارای exoSو 19 سویه (66/12%) دارای nan1 بودند. فراوانی ژن nan1 در ایزوله های زخم (30%) به صورت معنی داری بیشتر از ایزوله های سوختگی (4%) بود.
نتیجه گیری: به نظر میرسد که استفاده همزمان از پرایمرهای ژنهای oprIو oprL و toxA، حساسیت کافی را برای شناسایی سودوموناس آئروجینوزا از نمونه های کلینیکی فراهم میکند. فراوانی زیاد ژن exoSدر این ایزوله ها منعکس کننده توانایی آنها برای عفونت تهاجمی است. فراوانی ژن nan1در ایزوله های زخم نسبت به سوختگی بیانگر نقش احتمالی آن در عفونتهای زخم می باشد.
Background and Aim: In recent decades، Pseudomonas aeruginosa has emerged as one of the most important nosocomial pathogens. Due to the clinical importance this bacterium، various methods have been developed to rapidly and accurately identify it. The aim of this research was to detect P. aeruginosa isolated from wound and burn infections on the basis of the amplification of the oprI، oprL and toxA genes، and to determine the prevalence of nan1 and exoS genes among them.
Materials and Methods: A total of 150 P. aeruginosa isolates was collected from patients with burn and wound infections of Imam-khomaini، Tohid and Motahari hospitals in Tehran. The isolates were identified as P. aeruginosa using specific biochemical tests. Chromosomal DNA of the isolates was extracted with phenol chloroform method and used for PCR of oprI، oprL، toxA، exoS and nanI genes by specific primers.
Results: Among 150 P. aeruginosa isolates all carried the oprl and oprL genes; 98 (65.3%)، 142 (94.7%) and 19 (12.66%) of the isolates were positive for exoS، toxA and nanI genes respectively. The presence of nan1 gene in wound isolates (30%) was significantly higher (p<0.05) than in burn isolates (4%).
Conclusion: Our results indicated that simultaneous use of oprl، oprL and toxA genes provide sufficient sensitivity to detect P. aeruginosa in clinical samples. The high prevalence of exoS in isolates suggests invasive phenotype of wound and burn isolates. The high prevalence of nan1 in wound isolates suggests a possible role of this gene in those infections.
