چکیده:
سابقه و هدف: دیابت نوع 2 نوعی اختلال شایع متابولیک است که با افزایش خطر بیماریهای قلبی عروقی، ناتوانی و مرگ و میر ناشی از آن در مبتلایان همراه است. شواهد اپیدمیولوژیک و مطالعات بالینی طی سالهای اخیر همواره بر این مطلب تاکید داشته اند که پلی فنول های رژیم غذایی و مواد غذایی غنی از این فیتوکمیکال ها در پیشگری از بروز بیماریهای قلبی عروقی، به ویژه بیماری کرونر قلب و انفارکتوس میوکارد، تاثیر بسزایی دارند. مقاله مروری حاضر با هدف بررسی مکانیسم تاثیر پلی فنول های موجود در مواد غذایی طبیعی در پیشگیری از عوارض قلبی عروقی ناشی از دیابت صورت پذیرفت.
مواد و روشها: واژگان کلیدی «پلی فنول ها»، «اسیدهای فنولیک»، «آنتوسیانین ها»، «فلاونوئیدها»، و «لیگنان ها»، در ترکیب با واژگان «دیابت نوع 2»، «مقاومت به انسولین»، «بیماریهای قلبی عروقی»، «اختلال عملکرد اندوتلیال»، «استرس اکسیداتیو»، «التهاب»، «اختلال چربیهای خون» و «پرفشاری خون» جهت جستجو در پاب مد بین سالهای 2000 تا 2013 میلادی مورد استفاده قرار گرفت. مطالعات سلولی، مدل های حیوانی و مطالعات بالینی با طراحی مناسب و نیز مقالات مروری با کیفیت مطلوب استفاده شد.
یافته ها: پلی فنول های غذایی به واسطه کاهش هضم و جذب چربیهای موجود در رژیم غذایی، افزایش بازگردش و دفع کلسترول،کاهش سنتز آپولیپوپروتئین های آتروژنیک، و مهار سنتز کلسترول و تری گلیسریدها قدرت بالایی در بهبود فراسنجهای لیپیدی دارند. این ترکیبات نقش موثری در محافظت عروق در برابر آسیبهای اکسیداتیو و فرآیندهای التهابی، بهبود عملکرد اندوتلیال عروق، تعدیل فرآیندهای مرتبط با فعالیت پلاکت ها و انعقاد خون، و فراتر از آن حتی محافظت از بافت قلب و بهبود همودینامیک عضله قلب دارند.
نتیجه گیری: افزایش مصرف مواد غذایی حاوی پلی فنول و استفاده از عصاره طبیعی پلی فنول ها می تواند در کاهش خطر بروز و گسترش بیماریهای قلبی عروقی مبتلایان به دیابت نوع 2 مفید واقع گردد.
Background and Aim: Type 2 diabetes is a common metabolic disorder accompanied by increased risk of cardiovascular diseases، along with increased morbidity and mortality. In recent years، epidemiological evidence and clinical studies have indicated that dietary polyphenols and phytochemical-rich foods have key role in prevention of cardiovascular diseases، especially coronary heart disease and myocardial infarction. The current review was conducted to investigate the mechanism of the effects of polyphenols in natural foods on prevention of diabetes-induced cardiovascular diseases.
Materials and Methods: “polyphenols”، “phenolic acids”، “anthocyanins”، “flavonoids”، and “lignans” in combination with “type 2 diabetes”، “insulin resistance”، “cardiovascular disease”، “endothelial dysfunction”، “oxidative stress”، “inflammation”، “lipid disorders”، and “hypertension” were used as key words for search in PubMed between 1990 and 2013. Cellular studies، animal models and clinical trials with appropriate design، and review article with high quality were used.
Results: Dietary polyphenols have high potential to improve lipid profiles through decrease in digestion and ingestion of dietary fats، increase in turnover and excretion of cholesterol، reduction of synthesis of atherogenic apo lipoproteins، and inhibition of cholesterol and triglycerides synthesis. These compounds protect vascular endothelium against oxidative damages and inflammatory processes، improve function of vascular endothelium، modify the pathways related to platelets and coagulation، and even protect cardiac tissue and improve myocardial hemodynamics.
Conclusion: Higher consumption of foods containing polyphenols and using polyphenol-rich natural extract can be useful in decreasing the risk of development of cardiovascular diseases in type 2 diabetic patients.
