چکیده:
سابقه و هدف: شنیدن سیگنال های صوتی قلب یکی از روش های غیر تهاجمی و در عین حال آسان در تشخیص بی نظمی های عملکردی قلب انسان محسوب می شود که تحلیل صحیح آن، به دانش و تجربه ی پزشک متخصص نیاز دارد. هدف این مقاله، طراحی و پیاده سازی یک سیستم هوشمند در جداسازی و طبقه بندی آریتمی های موجود در سیگنال های صوتی قلب است.
مواد و روشها: این پژوهش در مرحله ی نخست به صورت اکتشافی و در مرحله ی دوم، به صورت تجربی پیاده سازی شده است. در گام اول، سیگنال های صوتی قلب از طریق دستگاه ضبط کننده ی صداهای قلبی (PCG) به دست آمدند. داده ها از 41 داوطلب به صورت آگاهانه و غیر مداخله ای در طول یک ماه، از مرکز مطالعات بالینی قلب و عروق بیمارستان واسعی شهر سبزوار، به صورت نرمالیزه شده دریافت شدند. با آزمایش نمونه های مورد مطالعه، 104 سیگنال طبیعی و 60 سیگنال صوتی غیر طبیعی قلب از مجموع افراد گردآوری شد. در مرحله ی پردازش اولیهی داده ها، با استفاده از فیلتر تطبیقی، نویزها و اختلالات محیطی، از سیگنال صوتی جداسازی می شوند. سپس با اعمال تبدیل CWT به داده ها، فرایند استخراج ویژگی بر اساس سیکل های قلبی سیگنال صوتی صورت گرفته و در نهایت بردارهای ویژگی 32 گانه موجک با استفاده از ضرایب جزئی، تشکیل می شود. طبقه بندی نهایی سیگنال های صوتی طبیعی و غیر طبیعی قلب، با استفاده از شبکه ی عصبی چند لایه پرسپترون و به صورت پس انتشار (MLP-BP) صورت می پذیرد. 70% داده ها در آموزش شبکه ی عصبی پیشنهادی و 30% آنها برای آزمایش شبکه MLP-BP به کار گرفته شدند.
یافته ها: پیاده سازی مرحله های جداسازی و طبقه بندی سیگنال های صوتی با استفاده از 15 بیمار قلبی که شامل 10 مرد و 5 زن با میانگین سنی 7/561 سال و 26 فرد سالم که شامل 19 مرد و 7 زن با میانگین سنی 7/85/56 سال انجام گرفت و مشاهده شد که به ترتیب دقت های 5/190/96 درصد و 94 درصد در خروجی الگوریتم حاصل آمدند. عملکرد قابل قبول سیستم، بر اساس طراحی دقیق نرم افزاری بوده و به همین دلیل کارایی مناسبی را در عمل به همراه دارد.
نتیجه گیری: بر مبنای قابلیت اطمینان به خروجی نرم افزار در طبقه بندی صحیح سیگنال های صوتی قلب و به دلیل انطباق بالای تشخیص در مرحله ی اکتشاف با تشخیص در واقعیت، امکان پیاده سازی این سیستم در بخش های بیمارستانی وجود داشته و انجام آن را توجیه می کند.
Background and Aim: Heart and cardiovascular diseases is considered as a major cause of increased mortality worldwide. Listening to the audio signal of the heart is effectively simple and non-invasive technique to detect the irregularities in heart function so that the correct analysis of it requires specialist knowledge and experience. The purpose of this paper is to design and implement an intelligent system for the isolation and classification of cardiac arrhythmias in the audio signal.
Materials and Methods: We received heart sound signals through the heart sound recorder (PCG) in this system. The database were obtained from 41 informed volunteers and non-intervention form during a one month period from Cardiovascular Clinical Research Center Hospital in Sabzevar Vasei which included 15 patients with heart disease (10 males and 5 females with a mean age of 61±5.7 years) and 26 healthy volunteers (19 males and 7 females with a mean age of 56.5±8.7 years) and finally the data were normalized. By testing the samples، 78 normal and 45 abnormal heart sound signals was selected from all subjects. By using adaptive filter، noise and environmental disturbances are extracted from the audio signals in pre-processing step of data. Feature extraction process will do based on cardiac cycles of sound signal in next step by applying the CWT transform on segmented signals and the 32-dimensions matrix of feature vectors will made up using wavelet coefficients component. The final classification of normal and abnormal heart sound signals is accomplished by using multi-layer perceptron neural network and back-propagation (MLP-BP). 70% and 30% of the data were used for training and testing the proposed neural network respectively based on our experiences.
Results: The proposed algorithm isolated original signal from noise signal and classified normal and abnormal sound signals at an acceptable level with an average accuracy of 96.90%±1.5% and 94%، respectively.
Conclusion: This system has the potential for implementation in residential homes for people with different ages due to the reliability of the output of software in correct classification of audio signals and finally a person would be aware of his heart health.
