چکیده:
سابقه و هدف: خود درمانی، یکی از مسایل مهم بهداشتی- اجتماعی است که می تواند معضلات بسیاری را برای فرد و جامعه ایجاد نماید. خوددرمانی به ویژه در زنان در دورانهای حساسی همچون بارداری و شیردهی از اهمیت مضاعفی برخوردار می باشد؛ زیرا تولد نوزاد ناهنجار، می تواند مشکلات خانوادگی و اجتماعی زیادی را ایجاد کند. این مطالعه با هدف تعیین تاثیر برنامه آموزش بهداشت بر اساس مدل اعتقاد بهداشتی بر رفتارهای پیشگیری کننده از مصرف خودسرانه داروها در مادران باردار شهر اراک صورت گرفت.
مواد و روش ها: مطالعه حاضر از نوع مداخله ای می باشد که در آن 200 نفر از مادران باردار مراجعه کننده به مراکز بهداشتی- درمانی شهر اراک شرکت داشته اند (نمونه ها به صورت تصادفی به دو گروه تجربی و شاهد، هر کدام 100 نفر، تقسیم شدند). نمونه گیری به صورت خوشه ای و بر اساس مناطق پستی شهر اراک صورت گرفت. روش گردآوری اطلاعات، پرسشنامه تنظیم شده بر اساس مدل اعتقاد بهداشتی (حساسیت، شدت، منافع و موانع درک شده، راهنمای عمل) و چک لیست عملکرد مادران در خصوص مصرف خودسرانه داروها بود. قبل از انجام مداخله آموزشی، پرسشنامه و چک لیست برای هر دو گروه تکمیل شد و سپس مداخله آموزشی برای گروه تجربی در طی یک ماه و در قالب 4 جلسه آموزشی 50 دقیقه ای انجام گردید. همچنین 1 و 2 ماه بعد از انجام مداخله آموزشی، 2 جلسه پیگیری برگزار شد؛ سپس 3 ماه پس از مداخله آموزشی، در هر دو گروه، اطلاعات مجددا گردآوری و تجزیه و تحلیل شدند.
یافته ها: میانگین سنی گروه تجربی و شاهد به ترتیب 7/4±2/29 و 8/3±1/27 سال و میانگین مدت زمان بارداری 6/4±2/7 هفته بود. قبل از مداخله آموزشی میزان آگاهی، حساسیت درک شده و عملکرد در زمینه خوددرمانی کمتر از حد متوسط و میزان شدت، منافع و موانع درک شده در زمینه مصرف خودسرانه داروها بیشتر از حد متوسط بود ولی پس از مداخله آموزشی بین دو گروه در همه متغیرهای ذکر شده اختلاف معنی داری ایجاد شد و عملکرد مادران نیز در زمینه خوددرمانی کاهش پیدا کرد.
نتیجه گیری: هر چه آگاهی، حساسیت، شدت و منافع درک شده بالاتر و موانع درک شده مادران باردار جهت مصرف خودسرانه داروها پایینتر باشد، عملکرد در زمینه خوددرمانی کمتر شده است. یافته های این بررسی تاثیر و کارایی مدل اعتقاد بهداشتی (در مقایسه با روشهای سنتی آموزش در درمانگاههای گروه شاهد) را در کاهش مصرف خودسرانه داروها در مادران باردار نشان می دهد؛ لذا پیشنهاد می گردد آموزش بر اساس این مدل در جهت کاهش سوء مصرف خودسرانه داروها در سایر مراکز بهداشتی نیز انجام گیرد.
Background & Aim: Self-medication is one of the most serious health problems inducing many problems for society and people. This occurs more in women، because this group may be in sensitive periods، for example pregnancy and milking; therefore، birth abnormal neonatal situations may cause many social and familial problems. The purpose of this study is to determine the effect of educational program based on Health Belief Model (HBM) on preventive behavior of self-medication in woman with pregnancy in Arak، Iran.
Materials & Methods: In this quasi-experimental study two hundred women with pregnancy، and referring to health centers participated. The subjects were randomly divided into two groups: the experimental and control groups (each group consisting 100 women). Sampling was done with clustering based on mailing address in the city. Data collection was performed with a questionnaire based on HBM (perceived susceptibility، severity، barrier and benefit، and cues to action and performance checklist about self-medication was used in order to evaluate the effectiveness of the educational program. Before intervention، data collection for both groups was conducted upon a questionnaire and checklist. Then، intervention was done in 1 month consisting of 4 sessions، each 50 minutes post intervention. 1 and 2 months after intervention، two follow up sessions were held، and 3 months after، data were recollected and analyzed.
Results: The average age of case and control groups were 29.2± 4.7 and 27.1± 3.8 years، respectively; and mean time of pregnancy was 7.2. ± 4.6 weeks. The results indicated that the mean scores of knowledge، perceived susceptibility and performance about self-medication was lower than average، while severity، barrier and benefit about self medication were in higher than average. The mean score of all HBM elements of experimental group changed significantly after the intervention and in comparison to the control group، and performance of women about self-medication reduced (p<0/05).
Conclusion: The more the knowledge، perceived susceptibility، severity and benefit are higher، the less will be the women performance about self medication. Findings of this research confirmed the effect on health education based on HBM (compared with educational traditional methods in control group health centers) on self medication in women with pregnancy. Therefore، proposed education based on this model is suggested to be performed in other health center for reducing this problem.
