چکیده:
نظر به گزارشات متفاوت از درمان آسیب های عصب رانی و بمنظور تعیین تاثیر اعمال جراحی بر ضایعات جنگی عصب رانی مراجعه کنندگان به بخش های جراحی بیمارستان سوانح و سوختگی و دانشگاه علوم پزشکی شهید بهشتی این تحقیق انجام گرفته است. تحقیق به روش Quasi Experimental روی 27 نفر از مجروحین جنگ تحمیلی انجام و سن، جنس، زمان بروز آسیب، ضایعات همراه و اعمال جراحی اضافی در پرونده بیماران ثبت گردید. بیماران بر حسب وضعیت عصب رانی به سه گروه تقسیم شدند: گروه اول به تعداد 18 نفر با قطع کامل عصب، گروه دوم به تعداد 4 نفر با امتداد عصب ولی پوشیده از بافتهای اسکلروزه و فیبروزه و تحت فشار و گروه سوم به تعداد 5 نفر که قسمتی از عصب سالم و قسمتی دیگر قطع شده بود. نتیجه عمل جراحی با وضعیت آتروفی عضلات و اندازه گیری قدرت عضلات باز کننده ساق روی ران و ران روی شکم در قبل و بعد از عمل براساس مقیاس بین المللی (M0-M5) تعیین گردید و قدرت عضله M3 تا M4 به عنوان نتیجه خوب و M5 به عنوان نتیجه عالی تلقی گردید. شیوع آسیب جنگی اعصاب محیطی یک درصد بود. افراد مورد بررسی یک زن و 26 مرد در سنین 4.4±21.4 سال و فاصله زمان حادثه تا معرفی بیمار 2.4±3.6 ماه بود، پس از عمل بیماران از 3 تا 8 سال پیگیری شدند. لاپاراتومی در 24، کلستومی در 10 و پیوند عروق در 3 مورد انجام گرفت. نتیجه عمل در گروه اول 94.4، در گروه دوم 75 و در گروه سوم 100 درصد عالی و در جمع سه گروه در 7.4 درصد خوب و 92.5 درصد عالی بوده و پس از ترمیم عصب، آتروفی عضلات تقریبا از بین رفت و بیماران به زندگی طبیعی خود بازگشتند. این بررسی نشان داد که اعمال جراحی بر ضایعات جنگی عصب رانی، دارای نتیجه خوب و عالی می باشد.
There has been various reports on femoral nerve injuries treatment. Together with experience gained from the effect of surgical operation on war injuries of femoral nerve which were referred to surgery department of Accident and Burn hospital and medical university of shaheed Beheshti, had composed the basis for the present investigation. The research method was quasi experimental and was conducted on 27 wounded of Iran-Iraq war and their age, sex, the time of wound appearing, accompanied injuries and additional surgical operations were recorded. Patients, according to their femoral nerve, were divided to three groups: 1) 18 with complete denervation, 2) 4 with nerve extension but covered with sclerosis and fibrotic tissues and under pressure and, 3) 5 whose nerve partly were intact whereas other part of their nerves were denervated, the result of surgery operation was determined by situation of muscle atrophy and measurement of power of extensor quadriceps and psoas muscles before and after operation on the basis of international scale (M0-M5). Muscle power from M3 to M4 were taken as good and M5 as excellent. Incidence of war injuries of peripheral nerves was 1%. One of the patients was female and the rest were male with the average age of 21.4±4.4 years, the period between accident time and introduction of patient were 3.6±2.4 months and after operation, they were followed-up for 3-8 years. In the history of the 27 patienst, 24 cases had laparotomy, in 10 cholostomy and in 3 vessels grafting was performed. The results of operation were; in the first group 94.4%, in the second group 75% and in the last 100% excellent and in all three groups, there were 7.4% good and 92.5% excellent. After nerve repair, Muscle atrophy nearly disappeard and patients were back to their normal life.
