چکیده:
با توجه به شیوع جراحی ها و عوارض شناخته شده بر اثر مصرف بیجای آنتی بیوتیک ها جهت جلوگیری از عفونتهای پس از جراحی و به منظور تعیین تاثیر تک دوز سفازولین به عنوان پیش گیری از این قبیل عفونتها، این مطالعه در بخش جراحی بیمارستان امام خمینی تهران انجام گرفت. پژوهش حاضر با روش کارآزمایی بالینی به صورت تصادفی و یک سوکور بر روی 200 بیمار که تحت جراحی های مختلف (تمیز یا تمیز آلوده شونده) قرار گرفتند، صورت پذیرفت و از نظر بروز عفونت پس از عمل بررسی شدند. جهت پیشگیری از عفونت پس از جراحی، 100 بیمار نیم تا یک ساعت قبل از عمل یک گرم سفازولین دریافت نمودند. این بیماران تا ترخیص از بیمارستان آنتی بیوتیک دیگری دریافت نکردند (گروه تجربی). صد بیمار دیگر بنابر توصیه جراح و روند عادی بخش جراحی، آنتی بیوتیک هایی را که طبق روال بخش پس از عمل جراحی تجویز می شد یک تا چند روز پس از عمل دریافت کردند (گروه شاهد). بر طبق آزمون های آماری هزینه مصرف آنتی بیوتیک در گروه تجربی به مراتب کمتر از گروه دوم و از نظر آماری معنی دار بود. از نظر شیوع عفونت در گروه اول، علایم عفونت در 6 بیمار مشاهده گردید که در 3 بیمار به صورت تورم و سرخی موضعی، در 2 بیمار به شکل تب و در 1 بیمار به صورت آبسه چرکی بروز نمود. در گروه شاهد، علایم عفونت در یک بیمار به شکل آبسه چرکی ملاحظه شد. پس از ترخیص از بیمارستان در یک بیمار از گروه تجربی ترشحات مختصری در ناحیه زخم جراحی به وجود آمد که بدون تجویز آنتی بیوتیک بهبود یافت و در دو بیمار از این گروه تورم و درد مختصر مشاهده گردید. یک بیمار از گروه شاهد دچار عفونت شدید شد که نیاز به تخلیه چرک از محل زخم و مصرف آنتی بیوتیک پیدا نمود و یک بیمار دچار تورم در ناحیه جراحی گردید که بین این دو گروه از نظر آماری اختلاف معنی داری وجود نداشت. براساس نتایج این مطالعه مصرف آنتی بیوتیک ها فقط به عنوان یک تک دوز نیم تا یک ساعت قبل از شروع عمل جراحی جهت پیشگیری از عفونت زخمهای پس از عمل توصیه می گردد. با توجه به به نتایج حاصل استفاده از تکدوز سفازولین سبب کاهش هزینه بیمار و مدت بستری و احتمالا جلوگیری از مقاومتهای باکتریایی می گردد.
Considering the prevalence of surgeries and known complications due to unnecessary use of antibiotics for preventing post-surgical infections, this study was performed in the surgery ward of Tehran’s Imam Khomeini Hospital, to determine the effect of single-dose cefazoline in preventing these infections. This clinical trial was a randomized single blind study performed among 200 patients who had undergone various (clean or clean contaminated) operations and were evaluated in terms of infection incidence after surgery. To prevent post-surgical infection, 100 patients received 1 gram cefazoline 30 minutes to one hour before the operation. These patients (case group) did not receive any other antibiotic during their admittance in the hospital. A hundred other patients (control group) received antibiotics one to few days after surgery, according to the surgeon’s prescription or the surgery ward routine. According to statistical tests, the cost of antibiotics in the case group was significantly lower than the control group. In terms of infection prevalence in the case group, signs of infection were seen in 6 patients (local swelling and erythema in three cases, fever in two and a purulent abscess in one). In the control group, infection was observed as a purulent abscess in one patient. After discharge from the hospital, in the case group, one patient complained of little discharge from the incision site, which recovered without antibiotic prescription, and two patients complained of slight pain and swelling. In the control group, one patient was afflicted by severe infection, which mandated aspiration of pus from the incision site and antibiotic use, and one patient had swelling at the site. No statistically significant difference was seen between the two groups. Based on study results, antibiotic use is recommended merely as a single dose, half to one hour before surgery, to prevent post-surgical incision site infection. Considering the results, the use of single-dose cefazoline leads to reduction in patient costs and admittance duration, and possibly prevents bacterial resistance.
