چکیده:
مقدمه: امروزه اندازه گیری و شناسایی ابعاد مختلف کیفیت خدمات جهت تخصیص موثر منابع و تضمین سطح بالاتری از رضایت بیماران در دستور کار مدیران مراقبت بهداشتی قرارگرفته است. مطالعه حاضر باهدف ارزیابی کیفیت خدمات از دیدگاه بیماران و مدیران در مراکز سرپایی تحت پوشش بیمارستان های دانشگاه علوم پزشکی ایران انجام شد.
روش ها: مطالعه حاضر از نوع توصیفی-تحلیلی و بهصورت مقطعی در سال 1398 انجام شد. تعداد 435 نفر بیمار و 15 نفر مدیر با روش تصادفی نظاممند در این مطالعه شرکت داشتند. داده های مطالعه با استفاده از پرسشنامه دارای روایی و پایایی تائید شده سروکوال جمعآوری شدند. برای تحلیل داده ها از آزمون های کای دو و یو-من-ویتنی استفاده شد. تمام تحلیل های آماری با استفاده از نرمافزار SPSS نسخه 25 انجام شد.
یافته ها: میانگین نمره انتظارات بیماران (76/ 4) بالاتر از مدیران (58/ 4) است، درحالیکه سطح ادراکات بیماران (20/ 3) از خدمات پایینتر از ادراکات مدیران (73/ 3) گزارش شد. شکاف بین انتظارات و ادراکات از دیدگاه بیماران در تمام ابعاد معنی دار شد، بهطوریکه بعد پاسخگویی (70/ 1-) بیشترین اختلاف میانگین نمره را داشت. از دیدگاه مدیران نیز اختلاف بین انتظارات و ادراکات در همه ابعاد به جز بعد پاسخگویی معنی دار گزارش شد و بعد محسوسات (73/ 1-) بیشترین اختلاف نمره را داشت.
نتیجهگیری: نتایج مطالعه نشان داد که بین انتظارات و ادراکات از دیدگاه بیماران و مدیران در تمام ابعاد کیفیت خدمات شکاف منفی وجود دارد. بنابراین، مدیران بیمارستان باید حداکثر تلاش خود را در جهت کاهش شکاف بین انتظارات و ادراکات با ارائه به موقع خدمات و بهبود مهارت های ارتباطی کارکنان بهکارگیرند.
Introduction: Nowadays, measuring the quality of services in order to effectively allocate resources and ensure a higher level of patient satisfaction is on the agenda of healthcare managers. The aim of this study was to investigate the possible gap between the perception and expectation of service quality from patients and managers’ perspective in the selected outpatient clinics affiliated with hospitals of Iran University of Medical Sciences.
Methods: This descriptive-analytical cross-sectional study was conducted in 2019. The required data were collected based on a valid and reliable questionnaire (SERVQUAL). Chi-square and Mann-Whitney U tests were used to analyze the data. The data were analyzed using SPSS version 25.
Results: The patientschr('39') expectation (4.76) was higher than managers’ expectation (4.58), while the patientschr('39') perception level of service quality (3.20) was lower than the managerschr('39') perception level (3.73). The gap between expectation and perception of patients in all dimensions was statistically significant; moreover, the highest gap was found in the responsiveness dimension. From the managerschr('39') viewpoint, a significant relationship was reported between the expectation and perception gap in all dimensions except the responsiveness one. Moreover, the highest mean score gap was related to the tangibility dimension (P-Value = -1.73).
Conclusion: There was a negative gap between the expectations and perceptions from managers and patients’ perspectives in all the service quality dimensions. Hospital managers should attempt to reduce the gap between the expectation and perception by providing timely services and improving staff communication skills.
