چکیده:
زمینه و هدف: استنشاق مولکول جوش شیرین (NaHco3)، در شکل نبولایزشده، در دستهای از التهابات سیستم تنفسی باعث کاستهشدن از شدت التهاب و بهبود نسبی وضعیت بیمار شده است. بیماری ویروسی COVID-19 ممکن است در سیر بالینی خود با فعالشدن فاکتورهای التهابی به سمت نارسایی تنفسی پیش برود و هنوز روشی برای کنترل آن یافت نشده است. در فضاهای مجازی توصیههایی مبنی بر انجام بخور سنتی جوش شیرین برای جلوگیری از پیشرفت علائم تنفسی عفونت COVID-19 مشاهده میشود. مطالعه حاضر به این سوال که آیا ممکن است بخور سنتی جوش شیرین در بهبود علائم تنفسی بیماران COVID-19 اثر بخش باشد، پرداخته است.
مواد و روشها: این مطالعه، یک مطالعه مروری است. جستجو با واژگان کلیدی در پایگاههای داده معتبر علمی و جمعآوری مطالب مرتبط با موضوع و نهایتا آنالیز کیفی روش انجام این مطالعه بوده است.
یافتهها: در فرمهای نبولایزشده داروها، داروها به شکل ریزقطره (آئروسل) وارد سیستم تنفسی میشوند. بسته به سایز ذرات، عوامل فیزیکی و فاکتورهای مربوط به سیستم تنفسی، محل اثر ذرات متفاوت است. در بخور سنتی (خانگی)، ریزقطره تولید نمیشود و بخار حاصل از تبخیر آب و ماده درون آن، چنانچه آن ماده قابلیت تبخیر در آن شرایط را داشته باشد، به سیستم تنفسی وارد خواهد شد. بر اساس برخی از مطالعات، استنشاق فرم نبولایزشده جوش شیرین، نه بخور سنتی آن، با مکانیسمهایی مانند افزایش PH، افزایش جریان خون راههای هوایی و کاهش چسبندگی خلط، به طور موفقیتآمیزی از شدت التهاب در سیستم تنفسی کاسته است. از تبخیر محلول جوش شیرین در آب طی بخور سنتی، آب و دیاکسید کربن حاصل میشود و بخار حاصله، حاوی مولکول NaHco3 نیست.
ملاحظات اخلاقی: در تدوین این مطالعه، اصل امانتداری و صداقت استناد به متون استفاده شده، و پرهیز از استنباط شخصی از تحلیلها رعایت شده است.
نتیجهگیری: از تبخیر محلول NaHco3 طی بخور سنتی (خانگی) جوش شیرین، مولکول بیکربنات سدیم متصاعد نمیشود، بلکه آب و دیاکسید کربن متصاعد میشود، لذا بخور جوش شیرین حداکثر مانند بخور آب گرم عمل میکند و اثرات ضد التهابی که از فرم نبولایز مولکول NaHco3 به دست میآید، از بخور آن به دست نمیآید. پس به لحاظ تئوریک، بخور جوش شیرین نمیتواند در التهابات تنفسی و عفونت COVID-19 اثربخش باشد. از سویی، استفاده از داروهای استنشاقی در COVID-19 ممکن است میزان انتقال آئروسلی عفونت را افزایش دهد، لذا به کارگیری فرم استنشاقی نبولایزشده جوش شیرین در مبتلایان به عفونت COVID-19 بایستی هم به لحاظ میزان اثربخشی و هم از جهت ایمنبودن، مورد تحقیق و بررسی قرار بگیرد. مطالعات بیشتر هم بر بخور هم نبولایز جوش شیرین در عفونت COVID-19 پیشنهاد میشود.
Background and Aim: Inhalation of nebulized baking soda (NaHco3) has been
ef fective for reducing the inf lammation intensity in some respiratory disorders.
COVID-19 infection may result in Acute Respiratory Distress Syndrome (ARDS)
through activation of inf lammatory factors and has no certain treatment. Some-times
in cyberspace, inhalation of baking soda vapor has been recommended to prevent
the development of COVID-19. The present study intends to answ er this question
that may inhalation of baking soda vapor be ef fective to control respiratory symptoms
in COVID-19 patients?
Materials and Methods: This is a review study. Searching key-w ords in valid
scientif ic databases and collecting contents related to the topic and f inally qualitative
analysis w as the method of this study.
Findings: Through using nebulized drugs, aerosols enter to respiratory tract.
According to particle size, some other physical characters and also respiratory tract
factors, particles deposition in respiratory tract is dif ferent. In traditional (homemade)
steam inhalation, aerosols are not produced and w ater vapor and vapor of other
soluble substances, if they have the ability to evaporate in thus situation, enters to
the respiratory tract. According to some studies, nebulized baking soda inhalation,
not vapor of its solution in w ater, has reduced respiratory tract inf lammation through
elevating airw ay pH, increasing airw ay blood f low and reducing sputum viscosity,
successfully. Evaporating of baking soda solution during traditional steam inhalation,
result in producing carbon dioxide and w ater and there is no NaHco3 molecule in
vapor.
Ethical Considerations: In compiling this study, the principle of f idelity and honesty
of quoting the used texts and avoidance of personal inference f rom analysis have
been observed.
Conclusion: No Sodium Bicarbonate molecule is produced f rom evaporating of
NaHco3 solution through traditional (homemade) steam inhalation of baking soda;
so, despite of nebulized baking soda w hich has NaHco3 molecule, evaporated
baking soda solution could not show anti-inf lammatory ef fects; so, theoretically,
steam inhalation of baking soda could not be ef fective in COVID-19 infection. On the
other hand, using inhaled drugs in COVID-19 infection may lead to further spread of
the disease. So, using inhaled nebulized baking soda in COVID-19 patients should
be studied for both its ef f icacy and its safety. More studies about both nebulized and
evaporated baking soda on COVID-19 infection are recommended
