چکیده:
زمینه و هدف: کمردرد یکی از شایعترین و پرهزینهترین مشکلات پزشکی در جوامع امروز است. هدف از مطالعهی حاضر تاثیر هشت هفته تمرین درمانی با ترکیبی از تمرینات پیادهروی و تقویت عضلات مرکزی در آب بر درد و نوسانات پاسچر در زنان مبتلا به کمردرد مزمن غیراختصاصی بود. روش بررسی: 20 زن مبتلا به کمردرد مزمن غیراختصاصی در دو گروه تجربی و کنترل در مطالعه حاضر شرکت کردند. بیماران در گروه تجربی هشت هفته (سه جلسه در هر هفته) تمرینات ترکیبی(پیادهروی و تقویت عضلات مرکزی) در آب را انجام دادند. بیماران درگروه کنترل در این مدت تحت درمان خاصی قرار نداشتند. قبل و بعداز مداخله درد و نوسانات پاسچر به ترتیب با استفاده از مقیاس سنجش بصری درد و دستگاه اندازهگیری فشار کف پایی ثبت شد. از نرم افزار SPSS-21برای تجزیه و تحلیل اطلاعات آماری استفاده شد(05/0p≤). یافتهها: نتایج بدست آمده در متغیرهای درد (000/0=p)، نوسانات قدامی خلفی (001/0=p)، محدوده نوسانات ( 02/0=p)، طول مسیر مرکز فشار( 008/0=p) و سرعت نوسانات( 009/0=p) پس از هشت هفته تمرین درمانی در گروه تجربی تفاوت معنیداری نشان داد. در متغیر نوسانات داخلی خارجی گروه تجربی و هیچ یک از متغیرهای گروه کنترل تفاوت معنیداری پس از گذشت هشت هفته مشاهده نشد( 05/0p˃). نتیجهگیری: با توجه به نتایج بدست آمده استفاده از پروتکل ترکیبی در محیط آب بر روی کاهش درد و نوسانات پاسچر در بیماران مبتلا به کمردرد مزمن غیراختصاصی موثر واقع شد. بنابراین استفاده از پروتکل مطالعه حاضر در ترکیب با محیط آب جهت بهبود تعادل و درمان این بیماران پیشنهاد میشود.
Background and aim: Low back pain is one of the most common and most expensive medical problems in modern societies. The aim of this study was to examine the effect of eight weeks walking and core strength training in water on pain and postural sway in women with non-specific chronic low back pain (NCLBP). Methods:twenty patients with NCLBP in both experimental and control groups (CG) participated in the present study. patients in the experimental group (EG) eight weeks (3 sessions per week) performed combined training (walking and core muscle strength training) in water. The patients in CG did not receive any training. Visual analogue scale and foot pressure system were used to evaluate the pain and postural sway in both groups before and after the intervention. All statistical analyses were performed using SPSS statistical software version 21 (p≤0.05). Results: The pain (p=0.000), anteroposterior center of pressure (CoP) direction sway (p=0.001), CoP sway area (p=0.02), path length (p=0.008) and CoP sway velocity (p=0.009) decreased significantly in EG. Mediolateral CoP direction sway in the EG and none of the variables in the CG did not show any significant differences after eight weeks (p˃0.05). Conclusion: According to the results, the use of combined protocol in the aquatic environment was effective in reducing pain and postural sway in patients with NCLBP. Due it is recommended to the use of the present study protocol in combination with the water environment to improve the pain and balance of these patients.
