چکیده:
اهداف: تکامل ابتدای کودکی بهعنوان یکی از حیطههای اولویتدار تعیینکنندههای اجتماعی سلامت است. دامنه وسیعی از متغیرها، تکامل کودک را تحت تاثیر قرار میدهند. هدف از این مطالعه، بررسی عوامل مرتبط با تاخیر تکامل در کودکان زیر 5 سال بود.
ابزار و روشها: مطالعه حاضر، یک مطالعه مورد- شاهدی بود که در فاصله فروردین تا مردادماه سال 1398 در شهرستان همدان انجام شد. در مجموع 462 کودک زیر 5 سال و مادرانشان شامل 231 کودک دارای تاخیر تکامل بهعنوان گروه مورد و 231 کودک سالم بهعنوان گروه شاهد وارد مطالعه شدند. عوامل خطر مورد بررسی شامل متغیرهای جمعیتشناختی و زمینهای، آگاهی مادران درباره تکامل کودکان و مجموعهای از رفتارهای مراقبتی بود. تحلیلهای آماری با استفاده از نرمافزار Stata 11 و آنالیز رگرسیون لجستیک انجام شد و سطح معناداری 5% در نظر گرفته شد.
یافتهها: نتایج نشان داد که رابطه سن مادر (1. 08-1. 00: CI95%، 1. 04 =OR)، سابقه بستری کودک در بیمارستان (4. 05-1. 63: CI95%، 2. 57 =OR)، مصرف مواد دخانی توسط والدین (3. 64-1. 20: CI95%، 2. 90 =OR) و میزان آگاهی مادر (0. 82-0. 25-: CI95%، 0. 44 =OR) با تاخیر تکامل کودکان، رابطه معنادار داشت.
نتیجهگیری: مادران دارای کودک زیر 5 سال، نیازمند دریافت آموزش در زمینه ابعاد مختلف تکامل و رفتارهای مراقبتی محافظتکننده کودکان در مقابل تاخیر تکامل هستند. شناسایی عوامل مرتبط با تاخیر تکامل کودکان کمتر از 5 سال، میتواند سیاستگذاران و برنامهریزان را در طراحی اقدامات موثر بهمنظور کاهش تاخیر در تکامل و عوارض آن یاری کند.
Aims: Early childhood development is one of the priority areas of social determinants of health. A wide range of factors influence a childchr('39')s development. This study aimed to investigate the factors associated with developmental delay in children under 5 years.
Instrument & Methods: This study was a case-control study that conducted from April to August 2019 in Hamadan city. 462 children under 5 years and their mothers participated (231 children that have a developmental delay in the case group and 231 healthy children in the control group). The risk factors assessed were demographic characteristics, mothers’ awareness, and childhood developmental care. The data analysis were performed using Logistic Regression by Stata11 software and a significant level of 5% was considered.
Findings: The results showed that there were statistically significant relation between the developmental delay with mothers’ age (OR=1.04, 95%CI:1.00-1.08), history of child hospitalization (OR=2.57, 95%CI:1.63-4.05), tobacco use by parents (OR=2.09, 95%CI:1.20-3.64), and mothers’ awareness (OR=0.44, 95%CI:-0.25-0.82).
Conclusion: Mothers with children under 5 years need to be educated on various aspects of development and caregiving behaviors that protect their children from developmental delays, and the Identify these factors can help planners to reduce developmental delays and their effects.
