چکیده:
مقدمه: اختلال استرس پس از سانحه (PTSD) نوعی اختلال ناهمگون است که افراد در معرض تروما را تحت تاثیر قرار میدهد. اگرچه ویژگیهای تشخیصی آن در کتابچه راهنمای تشخیصی و آماری اختلالات روانی، نسخه پنجم مجدداً طبقهبندی شده است، اما این اختلال با یادآوریهای بیش از حد و ناخواسته از تروما، همچنین اجتناب از یادآورهای تروما مشخص میشود. الگوهای شناختی و خلقوخوی منفی این اختلال و ناهمگونی علایم آن در افراد مختلف حاکی از وجود مکانیزمهای متعدد مغز و اعصاب در زمینه PTSD است. تحقیقات طی چند دهه گذشته، چندین راه بالقوه برای شناسایی نشانگرهای زیستی تشخیصی برای PTSD نشان داده است. این موارد شامل سیستمهای انتقالدهنده منوآدنرژیک، محور هیپوتالاموس-هیپوفیز-آدرنال، مسیرهای هورمونی متابولیک، مکانیزمهای التهابی، واکنشهای سایکوفیزیولوژی و مدارهای عصبی-مغزی است. بررسی حاضر، ادبیات پژوهشی را با توجه به امیدوارکنندهترین نشانگرهای زیستی در تشخیص PTSD با تاکید ویژه بر تعامل بین تاثیرات عصبی بر خطر بیماری و پیشرفت علایم ارایه میدهد. چنین نشانگرهای زیستی به احتمال زیاد توسط مدلهای چندبعدی حاصل از توضیحات جامع فنوتیپهای مولکولی، عصبی، رفتاری و بالینی شناسایی میشوند.
نتیجهگیری: بیومارکرهای تشخیص اختلال استرس پس از سانحه میتواند افقهای امیدوارکنندهای برای تشخیص و درمان به موقع این اختلال فراهم کند و ابزارهایی برای بالینگران فراهم کند تا تشخیصهای دقیقتری روی این اختلال هزارچهره بگذارند. همچنین ادبیات پژوهشی موجود در این زمینه میتوانند در کنار علایم PTSD به بسترهای بیولوژیکی مرتبط و اهداف مطالعات آینده خدمت کنند.
Introduction: Post-traumatic stress disorder (PTSD) is a heterogeneous disorder that affects people at risk for trauma. Although its diagnostic features have been reclassified in the Diagnostic and Statistical Manual of Mental Disorders, Version 5 (DSM-5), the disorder is characterized by excessive and unwanted trauma reminders as well as the avoidance of trauma reminders. Cognitive patterns and negative moods of this heterogeneity indicate multiple neurological mechanisms in the context of PTSD. Research over the past few decades has shown several potential ways to identify diagnostic biomarkers for PTSD. These include monoadrenergic transmission systems, the hypothalamic-pituitary-adrenal axis (HPA), metabolic, hormonal pathways, inflammatory mechanisms, psychophysiological reactions, and cerebral neural circuits. The present study presents the research literature concerning the most promising biomarkers in diagnosing PTSD with special emphasis on the interaction between neurological effects on disease risk and progression of symptoms. Such biomarkers are likely to be identified by multidimensional models derived from comprehensive descriptions of molecular, neurological, behavioral, and clinical phenotypes.
Conclusion: Post-traumatic stress disorder biomarkers can provide promising horizons for early diagnosis and treatment of the disorder and provide clinicians with tools to make more accurate diagnoses of the disorder. Also, the existing research literature in this field can serve the biological contexts related to PTSD symptoms and future studieschr('39') goals.
