چکیده:
هدف مطالعه حاضر مقایسه اثربخشی دو روش درمان حساسیت زدایی با حرکت چشم و پردازش مجدد و شمارش پیشرونده بر اضطراب، استرس و افسردگی بیماران جراحی بای پس عروق کرونر بود. طرح پژوهشی نیمه تجربی و از نوع پیش آزمون، پس آزمون و پیگیری با گروه کنترل بود. جامعه آماری شامل کلیه بیماران جراحی بای پس عروق کرونر مراجعه کننده به کلینیک بازتوانی مرکز قلب تهران بود، که نمونه ای به تعداد 36 نفر از طریق نمونه گیری در دسترس انتخاب و در گروههای آزمایش و کنترل قرار گرفتند. در گروههای آزمایشی، هر بیمار در 6 جلسه 45 دقیقه ای به صورت انفرادی تحت درمان قرار گرفت و در گروه کنترل هیچ مداخلهای صورت نگرفت. هر سه گروه مقیاس تجدید نظر شده تاثیر رویداد (ویس و مارمار، 1997) و مقیاس 21 سوالی افسردگی، اضطراب و استرس (لاویباند و لاویباند، 1995) را در قبل و بعد از مداخله و در پیگیری تکمیل نمودند. دادهها با استفاده از تحلیل کوواریانس چند متغیری تحلیل شد. نتایج مقایسههای زوجی بین گروهها هم در مرحله پس آزمون و هم در مرحله پیگیری نشانگر تفاوت معنادار بین گروه درمان حساسیت زدایی با حرکت چشم و پردازش مجدد و گروه کنترل، و گروه شمارش پیشرونده و گروه کنترل در متغیرهای افسردگی (فقط در مرحله پیگیری) و استرس (در هر دو مرحله پس آزمون و پیگیری) بود و این تفاوت بین گروهها در متغیر اضطراب مشاهده نشد. همچنین نتایج هیچ گونه تفاوت معناداری بین دو گروه آزمایش نشان نداد (01/0 p). بنابراین، روش درمان حساسیت زدایی با حرکت چشم و پردازش مجدد و روش شمارش پیشرونده در کاهش افسردگی و استرس در بیماران جراحی بای پس عروق کرونر موثر می باشند.
The purpose of this study was to compare the effectiveness of Eye Movement Desensitization and Reprocessing (EMDR) and Progressive Counting (PC) on anxiety, stress, and depression in patients with coronary artery bypass graft surgery (CABG). A quasi-experimental design was used to analyze the data which included a pre-test, post-test, and follow-up. The statistical population included all patients with coronary artery bypass graft surgery referred to the rehabilitation clinic of Tehran Heart Center and were selected using available sampling based on the inclusion and exclusion criteria. Thirty-six randomly selected patients were divided into two experimental groups and one control group (N=12 in each group). In the experimental group of EMDR, each patient underwent this treatment individually in six 45- minute sessions and the same was carried out for PC. The control group received no treatment. The research instruments included a revised version of the Impact of Event Scale (IES-R, Weiss & Marmar, 1997), Depression, Anxiety and Stress Scale (DASS-21, Lovibond and Lovibond, 1995), which were performed on patients in all three groups in the pre-test, post-test, and follow-up. Data analyzed using MANCOVA. The results showed that EMDR and PC were both effective in reducing depression and stress symptoms. Also, the results did not show any significant differences between the two experimental groups (P<0,01).
