چکیده:
تامین مالی حوزه سلامت و بهداشت در ایران از یک مدل چندگانه بهره میگیرد که در آن دولت، بیمهها و بیماران بهطور همزمان هزینههای بهداشتی و درمانی را متقبل میشوند. علیرغم مخارج گسترده دولت و پرداخت برخی هزینهها توسط بیمهها، سهم پرداخت از جیب بیماران قابل توجه بوده و لذا حمایت از بیماران امری ضروری است. یکی از روشهای تامین مالی بیماران اختصاص تسهیلات بانکی است که در بانکهای کشور از طریق قرضالحسنه انجام میگیرد. ازآنجاکه بانکها به دلیل حفظ منافعشان، تمایلی به اعطای تسهیلات قرضالحسنه ندارند عملا این شیوه از تامین مالی رونقی ندارد و در بسیاری از موارد بیماران با دریافت وامهایی غیر از درمان، به رفع نیازهای خود مبادرت میورزند که درواقع نوعی صوریسازی محسوب میشود. هدف از مقاله حاضر ارائه یک مدل بهمنظور رفع این نقیصه در نظام تامین مالی سلامت است. این مدل با استفاده از روش تحلیلی-توصیفی و مقایسه تطبیقی بهگونهای ارائه میشود که علاوهبر تامین منافع بانکها، پاسخگوی نیازهای درمانی متقاضیان تسهیلات نیز باشد. نتایج نشان میدهد تنوع نیازهای پزشکی در قالب خدمات تشخیصی، خدمات درمانی، خدمات پرستاری و پرستاری در منزل سبب شده است که به طیف متنوعی از عقود بانکی نیاز باشد. انطباق نیازهای پزشکی با عقود بانکی نشان میدهد که تسهیلات جعاله، مرابحه (فروش اقساطی)، استصناع و اجاره بهشرط تملیک میتواند در تامین نیازهای درمانی مورداستفاده قرار گیرد. سه بازیگر اصلی مدل بانک، بیمار و بیمارستان هستند و مکانیسم ارتباطی آن بهگونهای طراحی شده که از کژمنشی و کژگزینی جلوگیری نماید.
Abstract:Health care financing in Iran uses a multiple model in which the government, insurance companies and patients simultaneously bear the costs of health care. Despite the government's extensive expenditures and the payment of some expenses by insurance companies, the share of out-of-pocket payments is significant and therefore patient support is essential. One of the methods of financing patients is banking facilities, which is through Qarz al-Hasna. Because banks are reluctant to provide facilities through Qarz al-Hasna in order to protect their interests, this method of financing is practically unsuccessful, and in many cases patients receive other loans to meet their needs, which is a form of formalization. The purpose of this paper is to provide a model to address this shortcoming in the health financing system. This model is presented using analytical-descriptive method and comparative comparison in such a way that in addition to securing the interests of banks, it also meets the medical needs of facility applicants. The results show that the diversity of medical needs in the form of diagnostic services, medical services, nursing services and home nursing has led to the need for a diverse range of banking contracts. Compliance of medical needs with banking contracts shows that the facilities of Jaaleh, Murabaha (installment sale), Istisna and Ejareh can be used to meet medical needs. The three main player in the model are the bank, patient and the hospital, and its communication mechanism is designed to prevent moral hazard and adverse selection.
