چکیده:
مقدمه: مخارج سلامت عمومی یکی از عوامل مهم در بهبود وضعیت سلامت است. با توجه به اهمیت وضعیت سلامت در ارتقای شاخصهای توسعه انسانی و بهتبع آن افزایش رفاه اجتماعی و همچنین تاثیر عوامل نهادی و اجتماعی بر وضعیت رفاه اجتماعی، هدف اصلی این مطالعه بررسی تاثیر مخارج سلامت عمومی و کیفیت دموکراسی بر رفاه اجتماعی در کشورهای با شاخص توسعه انسانی بالا بود.
روش کار: مطالعه توصیفی- تحلیلی و کاربردی حاضر با روش رگرسیون پانلدیتا و برآوردگر خطاهای استاندارد تصحیح شده پانل برای سالهای 2000-2019 در سال 1399 انجام شد. جامعه آماری39 کشور با شاخص توسعه انسانی بالا بود و دادههای سری زمانی سالانه مطالعه از پایگاه دادهای بانک جهانی، راهنمای بینالمللی ریسک کشوری و سازمان ملل متحد استخراج شدند. مدل و آزمونهای موردنیاز تحقیق در نرمافزار Stata 16 برآورد شدند.
یافتهها: میانگین شاخص توسعه انسانی برای کل کشورهای نمونه طی دوره برابر با 04/0± 70/0، برای ایران برابر با 04/0 + 73/0بود و میانگین شاخص توسعه انسانی تعدیل یافته با نابرابری برای کل کشورهای نمونه برابر با 06/0±58/0 و برای ایران برابر با 08/0± 56/0 بود. متغیرهای مخارج سلامت عمومی به GDP، لگاریتم GDP، باز بودن تجاری و شاخص دموکراسی به ترتیب تاثیر مثبت به میزان 26/0، 02/9، 08/0 و 17/0 درصد بر شاخص توسعه انسانی داشتند همچنین متغیرهای مخارج سلامت عمومی به GDP، شاخص دموکراسی و حاصلضرب مخارج سلامت عمومی به GDP و شاخص دموکراسی به ترتیب تاثیر مثبت به میزان 34/0، 09/0 و 15/0 درصد بر شاخص توسعه انسانی تعدیل یافته با نابرابری داشتند.
بحث: نابرابری در ابعاد توسعه انسانی در ایران نسبت به سایر کشورهای با شاخص توسعه انسانی بالا، بیشتر بود و مخارج سلامت عمومی و کیفیت دموکراسی تاثیر مثبت بر رفاه اجتماعی داشتند. همچنین بهبود کیفیت دموکراسی موجب افزایش کارایی مخارج سلامت عمومی بر وضعیت رفاه اجتماعی شد؛ لذا سیاستهای افزایش مخارج سلامت عمومی در راستای سرمایهگذاری بیشتر در زیرساختها و افزایش سطح خدمات سلامت در مناطق مختلف جهت از بین بردن نابرابری، بهبود و ارتقاء فنّاوریهای حوزه سلامت که به مخارج عمومی وابسته است، پیشنهاد میگردد همچنین افزایش شفافیت و پاسخگویی دولت به مردم جهت افزایش رفاه اجتماعی و کارایی بیشتر مخارج سلامت پیشنهاد میگردد.
Introduction: Public health expenditures are one of the important factors in improving health status. Considering the importance of health status in promoting human development indicators and consequently increasing social welfare as well as the impact of institutional and social factors on social welfare status, the main purpose of this study was to investigate the impact of public health expenditures and the quality of democracy on social welfare in countries with high human development index.
Methods: This descriptive-analytical and applied study was performed by Panel data with Panel-Corrected Standard Errors (PCSE) estimator for the years 2000-2019 in 2021. The statistical population was 39 countries with a high human development index and the annual time series data were extracted from the databases of the World Bank, the International Country Risk Guide and the United Nations. The models and required tests were estimated in Stata 16 software.
Findings: The mean of human development index for all sample countries during the period was 0.70 ± 0.04, for Iran was 0.73 ± 0.04 and the mean of Inequality-adjusted Human Development Index for all sample countries was to 0.58 ± 0.06 and for Iran was equal to 0.56 ± 0.08. The variables of public health expenditures to GDP, GDP logarithm, Openness and democracy index had a positive effect of 0.26, 9.02, 0.08 and 0.17% on the human development index, respectively. Also, the variables of public health expenditures to GDP, Democracy index and the product of public health expenditures to GDP and democracy index had a positive effect of 0.34, 0.09 and 0.15% on the Inequality-adjusted Human Development Index, respectively.
Discussion: Inequality in human development in Iran was higher than other countries with high human development index, and public health expenditures and the quality of democracy had a positive effect on social welfare. Improving the quality of democracy also increased the efficiency of public health spending on social welfare; therefore, policies to increase public health expenditures are proposed in order to more invest in infrastructure and increase the level of health services in different regions to eliminate inequality, improve and upgrade health technologies that depend on public expenditures. It is also proposed to increase government transparency and accountability to the people in order to increase social welfare and increase the efficiency of health expenditures.
