چکیده:
زمینه و هدف: اوتیسم اختلالی عصب روانشناختی است که به صورت ثانویه منجر به مشکلات در عملکردهای شخصی، اجتماعی، و تحصیلی می شود. از این جهت پژوهش حاضر با هدف اثربخشی برنامه مداخله ای مبتنی بر آموزش ارتباط کارکردی در رفتارهای کلیشهای، رفتارهای کشاکش برانگیز، و آسیب به خود کودکان با اختلال طیف اوتیسم انجام شد.
روش: پژوهش حاضر به لحاظ هدف، کاربردی و از نظر روش جمع آوری اطلاعات از نوع آزمایشی با طرح پیشآزمون- پسآزمون - پیگیری با گروه گواه بود. جامعه آماری این پژوهش تمامی کودکان با اختلال طیف اوتیسم سطح یک 4 تا 8 ساله شهر تهران در سال 1398 بودند که از جامعه آماری مورد نظر، تعداد 30 کودک به صورت نمونه گیری هدفمند از کلینیک پدیده تهران، انتخاب و به صورت تصادفی در دو گروه آزمایش و گواه جایدهی شدند. سپس گروه آزمایش برنامه درمانی را در قالب 15 جلسه دریافت کرد. ابزار گردآوری اطلاعات شامل مقیاس رتبه بندی اوتیسم گیلیام (جیمز و گیلیام، 2003)، پرسشنامه غربالگری اوتیسم (اهلر و گیلبرگ، 1993)، مقیاس رفتار کشاکش برانگیز کودک (بیورکر تایلور و همکاران، 2010)، و پرسشنامه مشکلات رفتاری (روژان و همکاران، 2001) بود. تحلیل داده ها با استفاده از تحلیل کوواریانس در محیط SPSS26 انجام شد.
یافته ها: نتایج آزمون تحلیل کوواریانس یک و چندمتغیری نشان داد که تفاوت معناداری بین گروه آموزش و گواه به ترتیب در متغیر رفتارهای کشاکش برانگیز و مؤلفههای مشکلات رفتاری وجود دارد (0/001P). اندازه اثر برای متغیر رفتارهای کشاکش برانگیز (0/922) و مؤلفههای رفتارهای کلیشه ای (0/850) و رفتارهای آسیب به خود (0/832) بود. نتایج آزمون تی همبسته نیز نشان داد تأثیر برنامه مداخله ای در مرحله پیگیری نیز در بهبود رفتارهای کشاکش برانگیز و مؤلفههای مشکلات رفتاری، اثربخش و پایدار بوده است.
نتیجه گیری: با توجه به یافته های پژوهش می توان نتیجه گرفت که از برنامه درمانی مبتنی بر آموزش ارتباط کارکردی می توان به عنوان مداخه ای مناسب در کاهش نشانگان بالینی کودکان با اختلال طیف اوتیسم استفاده کرد.
Background and Purpose: Autism is a neuropsychological disorder that causes secondary impairment in personal, social, and academic functioning. This study aimed to develop an effective intervention program based on functional communication training (FCT) on stereotyped behaviors, challenging behaviors and self-harming behaviors in children with autism spectrum disorder (ASD).
Method: This was an applied experimental research with pretest-posttest-followup design with a control group. The statistical population of this study included all children with level one ASD, aged 4 to 8 years in Tehran in 2019, From which 25 children were selected by purposive sampling from Padideh Clinic in Tehran and randomly assigned to the experimental and the control groups. Then the experimental group received the intervention program in fifteen sessions. The instruments of the study included the Gilliam Autism Rating Scale (GARS-2) (James and Giliam, 2003), the Autism spectrum Screening Questionnaire (ASSQ) (Ehlers and Gillberg, 1993), the Child’s Challenging Behavior Scale (CCBS) (Bourke‐Taylor et al, 2010) and the Behavior Problems Inventory (BPI-01) (Rojahn et al, 2001). Data were analyzed by the analysis of covariance in the SPSS statistics-26 software.
Results: The results of the univariate and multivariate analysis of covariance showed that there was a significant difference between the experimental and the control groups in the variables of challenging behaviors and components of behavioral problems (P
Conclusion: According to the research findings, it can be concluded that a treatment program based on the FCT can be used as an intervention to reduce the clinical symptoms of children with ASD. The results of this study can benefit therapists and educators who are in contact with children with ASD.
