چکیده:
اهداف: فرتوتی با افزایش آسیب پذیری نسبت به عوامل استرس زا مشخص میگردد. بیماران مسن فرتوت در معرض افزایش ریسک پذیرش در بخش اورژانس، بستری شدن در بیمارستان و ناتوانی قرار دارند. مطالعه حاضر با هدف تعیین ارتباط سندرم فرتوتی و پلی فارمسی در سالمندان مراجعه کننده به بخشهای اورژانس بیمارستانهای آموزشی درمانی شهر اردبیل انجام شد.
مواد و روش ها: در این مطالعه توصیفی تحلیلی، 505 سالمند مراجعه کننده به اورژانس با روش نمونهگیری در دسترس انتخاب شدند. سندرم فرتوتی با استفاده از شاخصهای پنجگانه fried و همکاران که شامل: 1) سرعت کم راه رفتن 2) ضعف عضله 3) خستگی 4) فعالیت فیزیکی کم و 5) کاهش وزن غیر عمدی بود، سنجیده شد. پلی فارمسی نیزبا مصرف 5 دارو یا بیشتر در سالمندان تعیین گردید. دادهها با استفاده از روشهای آمار توصیفی شامل میانگین، انحراف معیار و استنباطی شامل تحلیل واریانس، آزمون تی مستقل، همبستگی و رگرسیون چندگانه به کمک نرم افزار آماری SPSS 22 تحلیل شد.
یافته ها: میانگین سنی شرکت کنندگان 7/49&+70/91بود. 255 نفر (50/5 درصد) مرد، 318 نفر (63 درصد) متاهل و 271 نفر (53/7درصد) بیسواد بودند. تعداد 204 نفر (40/4درصد) از سالمندان مراجعه کننده به اورژانس دچار فرتوتی و 177 نفر (35 درصد) پیش فرتوت بودند. تعداد 98 نفر (42/2 درصد) از سالمندان فرتوت و تعداد 86 نفر (37/1 درصد) از سالمندان پیش فرتوت به پلی فارمسی مبتلا بودند. آزمون همبستگی نشان داد بین فرتوتی و پلی فارمسی ارتباط معنی داری وجود دارد (0/001˂p). آنالیز رگرسیون خطی متغیرهای سن، تحصیلات، ترتیبات زندگی، بستری شدن دربیمارستان و تعداد داروهای مصرفی بیمار را به عنوان پیش بینی کنندههای سندرم فرتوتی در سالمندان مراجعه کننده به اورژانس نشان داد.
نتیجه گیری: تعداد داروهای مصرفی، سن، تحصیلات، ترتیبات زندگی ودفعات بستری در بیمارستان از عوامل پیش گویی کننده سندرم فرتوتی در سالمندان بودند. در نظر داشتن شرایط زندگی سالمندان و پیشگیری از بستریهای غیرضروری در بیمارستان میتواند باعث کاهش پلی فارمسی و درنهایت یک استراتژی پیشگیرانه در جهت پیشگیری و مدیریت فرتوتی باشد. البته برای تاییدمزایای احتمالی کاهش چنددارویی درایجاد، برگشت یا تاخیردرفرتوتی، تحقیقات بیشتری لازم است.
Objectives: Frailty is characterized by an increased vulnerability to stressors. Frail older patients are at increased risk of Emergency Department (ED) visits, hospitalization, and disability. The aim of this study was to determine the relationship between frailty syndrome and polypharmacy in the elderly referred to the emergency department of Ardabil teaching hospitals.
Methods & Materials: In this cross-sectional descriptive study, 505 elderly people referred to the hospital emergency department were selected by convenience sampling method. Frailty syndrome was measured using the five-item criteria of fried which include: 1) slow walking, 2) muscular weakness, 3) exhaustion, 4) low physical activity and 5) unintentional weight loss. Polypharmacy considered the use of 5 or more drugs. Data were analyzed using descriptive statistics including mean and standard deviation and inferential analysis including analysis of variance, independent t-test, correlation, and multiple regression using spss 22 statistical software.
Results:The mean age of participants was 70.91 ± 7.49 and 255 (50.5%) were male, 318 (63%) were married and 271 (53.7%) were illiterate. the results of the study showed that 204 (40.4%) of the elderly who referred to the emergency department had frailty and 177 (35%) had pre frailty. In this population, we identified a 42.2% (n=98) of frail with polypharmacy, 37.1% (n= 86) of prefrail with polypharmacy. The correlation test showed that there is a significant relationship between fertility and polypharmacy (p ˂0.001). Linear regression analysis showed the variables of age, education, living arrangement, hospitalization, and the number of medications used by the patient as predictors of frailty syndrome in the elderly referred to the emergency department.
Conclusions:Number of drugs used, age, education, living arrangement and number of hospitalizations were predictors of frailty syndrome in the elderly. Considering the living conditions of the elderly and the prevention of unnecessary hospitalizations can reduce polypharmacy and ultimately a preventive strategy to prevent and manage frailty. Of course, further research is needed to confirm the possible benefits of reducing polypharmacy in the development, reversion, or delay of frailty.
