چکیده:
مقدمه: هدف این مطالعه بررسی اثربخشی درمان مبتنی بر پذیرش و تعهد بر پریشانی هیجانی و پایبندی به درمان در بیماران دیابتی مبتلا به افسردگی است. روش: مطالعه حاضر یک مطالعه نیمهتجربی با طرح پیشآزمون - پسآزمون با گروه گواه و مرحله پیگیری سه ماهه بود. جامعه مورد مطالعه شامل افراد مبتلا به دیابت نوع 2 مبتلا به افسردگی مراجعهکننده به مراکز درمانی شهر اصفهان در شش ماهه اول سال 1400 بود. در این مطالعه 33 بیمار دیابتی افسرده با روش نمونهگیری داوطلبانه و هدفمند انتخاب و بهصورت تصادفی در دو گروه آزمایش و گواه گمارش شدند. گروه آزمایش به مدت 10 هفته تحت درمان مبتنی بر پذیرش و تعهد (10 جلسه) قرار گرفتند. پرسشنامههای مورد استفاده در این مطالعه شامل پرسشنامه افسردگی (بک و همکاران، 1996) (DQ)، پرسشنامه پریشانی هیجانی (کسلر و همکاران، 2002) (EDQ) و پرسشنامه پایبندی به درمان (موریسکی، انگ، کراسول -وود و وارد، 2008) (TCQ) بود. دادهها با استفاده از تحلیل واریانس آمیخته و آزمون تعقیبی بونفرنی با بهرهگیری از نرمافزار آماری SPSS23 مورد تجزیهوتحلیل قرار گرفت. یافتهها: نتایج نشان داد که درمان مبتنی بر پذیرش و تعهد بر افسردگی، پریشانی هیجانی و پایبندی به درمان در بیماران دیابتی افسرده تأثیر معناداری داشته است (0001>P) و توانسته افسردگی و پریشانی هیجانی را کاهش داده و پایبندی به درمان را در این بیماران بهبود بخشد. نتیجهگیری: بر اساس یافتههای حاصل از پژوهش حاضر، درمان مبتنی بر پذیرش و تعهد با بهرهگیری از شش فرایند اصلی پذیرش، گسلش، خود بهعنوان زمینه، ارتباط با زمان حال، ارزشها و عمل متعهدانه، میتواند روشی موثر برای کاهش پریشانی هیجانی و بهبود پایبندی به درمان بیماران دیابتی مبتلا به افسردگی باشد.
Objective: The present study was conducted to investigate the efficacy of acceptance and commitment based therapy on the emotional distress and Commitment to Treatment in diabetic patients with depression. Method: The present study was quasi-experimental with pretest, posttest, control group and three-month follow-up period design. The statistical population included type-2 diabetic people with depression who referred to therapy centers of Isfahan in spring and summer of 2022. 33 diabetic patients with depression were selected through purposive sampling method and were randomly accommodated into experimental and control groups. The experimental group received acceptance and commitment based therapy sessions (Ten sessions) during Ten weeks. The applied questionnaires included Depression Questionnaire (DQ) (Beck, Epstein, Brown, Steer, 1996) Emotional Distress Questionnaire (EDQ)(Kessler, et.al, 2002) and Commitment to Treatment Questionnaire (TCQ) (Morisky, Ang, Krousel-Wood, Ward, 2008). The data from the study were analyzed through mixed ANOVA and Bonferroni follow-up test using SPSS 23 statistical software. Findings: The results showed that acceptance and commitment based therapy has significant effect on the depression, emotional distress and Commitment to Treatment in diabetic patients with depression (pConclusion: According to the findings of the present study, ACT can be used as an efficient method to decrease emotional distress and improve Commitment to Treatment in the diabetic patients with depression through employing six main processes of acceptance, fusion, observer self, relationship with the present time, value and committed action.
